Chapter 6: Exercise and Physical Activity

The human skeleton is designed to respond to movement. Every step taken, every muscle contraction, and every weight-bearing activity sends signals that stimulate bone formation. Without regular movement, bones gradually lose density and strength, increasing the risk of osteoporosis and fractures.

Many people mistakenly believe that bones are fixed structures that cannot change after adulthood. Scientific research has shown the opposite. Bones are living tissues that continuously remodel themselves in response to physical demands. When muscles pull on bones during walking, climbing stairs, dancing, or lifting weights, the skeleton experiences small amounts of healthy stress. Bone-forming cells respond by strengthening the areas that receive regular mechanical loading. This adaptive process explains why active individuals generally have stronger bones than those who lead sedentary lifestyles.

Exercise offers benefits that extend far beyond improving bone density. It strengthens muscles, enhances balance, improves coordination, increases flexibility, supports cardiovascular health, helps maintain a healthy body weight, improves mood, reduces stress, enhances sleep quality, and promotes overall well-being. Weight-bearing exercise is considered the foundation of bone health. These activities require the body to work against gravity while supporting its own weight. Every step taken during walking places controlled stress on the hips, spine, and legs, encouraging bones to maintain or increase their strength. Weight-bearing exercises include walking, brisk walking, hiking, jogging, dancing, stair climbing, tennis, badminton, and low-impact aerobic exercise. The intensity should match an individual’s age, health status, and physical abilities.

Walking remains one of the safest and most accessible forms of exercise for most people. It requires no expensive equipment and can be performed almost anywhere. Regular walking strengthens the lower limbs, improves cardiovascular fitness, enhances circulation, and helps preserve bone density. For beginners, even a twenty-minute walk several days each week provides meaningful health benefits. As endurance improves, walking duration and pace can gradually increase under the guidance of healthcare professionals when necessary.

Brisk walking provides greater mechanical stimulation than casual walking because it places slightly more stress on the skeleton. Individuals who can safely walk at a faster pace often experience additional improvements in cardiovascular health, muscle endurance, and calorie expenditure. However, safety always takes priority. Proper footwear, well-maintained walking surfaces, and attention to environmental hazards reduce the risk of falls.

Stair climbing is another excellent weight-bearing activity. Ascending stairs requires greater effort from the muscles of the hips, thighs, and calves than walking on level ground. This increased muscle activity stimulates bone remodeling while improving lower-body strength and balance. Because movements are gentle and low-impact, many individuals with osteoporosis find Tai Chi both safe and enjoyable.

Yoga can also support bone and muscle health when practiced appropriately. Gentle yoga improves flexibility, posture, balance, breathing, and mental well-being. However, certain poses involving deep forward bending, twisting of the spine, or excessive spinal flexion may increase fracture risk in individuals with advanced osteoporosis. For this reason, yoga programs should be modified under the guidance of qualified instructors familiar with osteoporosis safety principles.

Flexibility exercises maintain joint mobility and reduce muscle stiffness. Gentle stretching before and after exercise helps improve range of motion and reduces discomfort. Stretching should always be performed slowly and without bouncing. Individuals with osteoporosis should avoid aggressive stretches that place excessive pressure on the spine or joints.

Swimming is often recommended for cardiovascular fitness because it places minimal stress on the joints. Although swimming provides excellent benefits for heart health, muscle endurance, and flexibility, it is not considered a weight-bearing exercise because the water supports much of the body’s weight. Therefore, swimming alone is usually insufficient to maintain bone density. Ideally, it should be combined with walking, resistance training, or other weight-bearing activities.

Cycling offers similar advantages and limitations. It improves cardiovascular fitness and leg muscle strength while placing relatively little stress on the joints. Outdoor cycling also promotes balance and coordination, although individuals with osteoporosis should consider the risk of falls. Stationary bicycles provide a safer alternative for many older adults but should be combined with weight-bearing exercises for optimal bone health.

Posture exercises become increasingly important as people age. Strong back muscles help maintain an upright posture, reducing the tendency toward spinal curvature associated with vertebral compression fractures. Exercises that strengthen the upper back, shoulders, and core muscles improve spinal alignment and decrease strain on weakened vertebrae. Good posture also enhances breathing efficiency and reduces fatigue during daily activities.

Core strengthening supports the spine by improving stability and balance. Gentle abdominal and back exercises help protect the vertebral column during movement. Daily physical activity extends beyond formal exercise sessions. Reducing prolonged sitting is particularly important because extended periods of inactivity accelerate muscle weakness and decrease bone stimulation.

Exercise programs should always be individualized. Age, existing medical conditions, previous fractures, physical fitness, medications, and overall health influence the most appropriate exercise plan. Individuals newly diagnosed with osteoporosis should consult healthcare providers or physical therapists before beginning vigorous exercise, especially if they have experienced spinal or hip fractures.

Safety is essential when exercising with osteoporosis. High-impact activities such as jumping, contact sports, and exercises involving sudden twisting or rapid changes in direction may increase fracture risk in individuals with severe osteoporosis. Heavy lifting with poor technique should also be avoided. Instead, exercises should emphasize controlled movements, gradual progression, proper posture, and good balance.

Proper footwear significantly reduces the likelihood of falls during exercise. Shoes should provide good arch support, stable soles, adequate cushioning, and secure traction. Loose slippers, worn-out shoes, and high heels increase fall risk and should generally be avoided during physical activity.

Warm-up and cool-down periods are important components of every exercise session. Five to ten minutes of gentle walking and stretching prepare muscles and joints for activity while reducing injury risk. Cooling down gradually allows heart rate and breathing to return to normal while preventing dizziness after exercise.

Motivation often presents a challenge, particularly for individuals who have never exercised regularly. Choosing enjoyable activities increases the likelihood of developing lifelong exercise habits.

Pain should never be ignored during exercise. Mild muscle soreness after new activities is common, but sharp pain, sudden back pain, chest pain, dizziness, or difficulty breathing require immediate medical attention. Listening to the body’s signals and progressing gradually helps prevent injuries while maximizing long-term benefits.

Exercise also supports emotional well-being. Regular physical activity reduces symptoms of anxiety and depression, improves self-confidence, enhances cognitive function, and promotes social interaction. Many individuals with osteoporosis experience fear of falling after a fracture. Carefully supervised exercise helps rebuild confidence, restore independence, and improve quality of life.

Scientific evidence consistently demonstrates that exercise remains beneficial even after osteoporosis has been diagnosed. Although existing bone loss cannot always be completely reversed, regular physical activity slows further bone deterioration, strengthens muscles, improves balance, reduces falls, and enhances overall health. Combined with proper nutrition and appropriate medical treatment, exercise forms one of the three pillars of osteoporosis management.

Developing a lifelong habit of regular movement is one of the greatest gifts individuals can give themselves. It is never too early to begin strengthening bones, and it is rarely too late to experience meaningful improvements. Whether through walking, resistance training, Tai Chi, gardening, or simply becoming more active throughout the day, every movement contributes to healthier bones and a more independent future.
